This follow-up will track people from the STONE cohort to see if changes in metabolic risk factors over time relate to heart and kidney outcomes.

What this trial studies
Researchers will re-contact the >4,000 adults enrolled in the STONE cohort in 2016–2017 and repeat questionnaires, physical exams, labs, and imaging to update cardiorenal outcomes. Participants will be grouped by CKM staging and metabolic risk profiles to compare current findings with baseline measures. The planned data collection for the cohort begins May 2025 with population-level data finalized by June 2026 and extended follow-up through 2035. The protocol excludes those who moved away or cannot be contacted and relies on visits at Shanghai Changzheng Hospital for testing.
Who should consider this trial
Good fit: Ideal candidates are adults from the original STONE cohort who completed the 2016–2017 baseline exams, remain contactable, and consent to follow-up.
Not a fit: People who moved away, cannot be reached after multiple attempts, or were excluded at baseline for serious illnesses are unlikely to benefit from participation.

Inclusion Criteria: 1. The 4,094 residents of Shanghai communities who have been enrolled in the STONE cohort. 2. Individuals who completed the baseline questionnaire survey, physical examination, and related clinical tests between 2016 and 2017, with complete data records. 3. Willingness to participate in this follow-up study and provision of informed consent. Exclusion Criteria: 1. Participants who have moved away from their original community and have been living elsewhere for more than two years. 2. Participants whose contact information has changed or become invalid, resulting in loss of contact (those who remain uncontactable after three attempts to reach out).
