What this trial studies

Inspire HER is a 24-week lifestyle program adapted from Black Impact that combines regular physical activity, nutrition education, and connections to health coaches and community health workers. The team will refine the program using prior feedback and run a randomized, wait-list–controlled trial comparing Inspire HER to usual care among Black women with elevated cardiovascular-kidney-metabolic risk. The study will measure changes in heart-health metrics and biological responses to stress as primary outcomes, and will track self-reported stress, perceived health control, and unmet social needs as secondary outcomes. Investigators will also compare women's responses to existing Black Impact data in men to explore sex differences in stress and heart-health pathways.

Who should consider this trial

Good fit: Ideal candidates are English-speaking Black women aged 30–79 who live in the metropolitan Birmingham, Alabama area and have stage 2 or greater cardiovascular-kidney-metabolic syndrome with no provider-imposed limits on physical activity.

Not a fit: People who are not Black women, live outside the Birmingham area, have provider-imposed limits on physical activity, or do not have elevated cardiometabolic risk are unlikely to benefit from participating.

Why it matters

Potential benefit: If successful, Inspire HER could lower cardiovascular risk and improve stress-related health and access to social supports for Black women.

How similar studies have performed: A similar program (Black Impact) showed promising improvements in heart health and stress-related outcomes in Black men in earlier studies, and a larger Black Impact trial is ongoing.

Inclusion Criteria:

* Black women (self-report)
* Adult ages 30-79 years
* Stage 2 or greater Cardiovascular-Kidney-Metabolic Syndrome
* English speaking
* Lives in Metropolitan Birmingham, AL area.

Exclusion Criteria:

\- Healthcare provider-imposed physical activity limitations.
