What this trial studies
This study tests a novel implementation program aimed at increasing access to tobacco control services for rural adults who smoke but are not yet ready to quit. It employs a hybrid approach that combines implementation and effectiveness outcomes, utilizing community paramedicine and mobile health technology. The trial will compare a standard implementation program with an enhanced version that includes an EMS Champion program, focusing on counties with high smoking rates in rural areas. Participants will receive support through a mobile health-assisted brief abstinence experience alongside nicotine replacement therapy.
Who should consider this trial
Good fit: Ideal candidates for this study are adults aged 18 and older who are current smokers, not yet ready to quit, and can engage with mobile health technology.
Not a fit: Patients who are actively quitting smoking or preparing to quit within the next 30 days will not benefit from this study.

Inclusion Criteria: * Current Smoker * at least 18 years old * speaks English * Able to receive texts and read text (literate) * Have a text-enabled phone or receive one from the study team * Not yet ready to quit smoking Exclusion Criteria: * actively quitting smoking * preparing to quit smoking within 30 days * prisoners * FDA- defined NRT contraindications * unable or unwilling to provide informed consent
