What this trial studies
This is a two-arm randomized trial enrolling 408 Latino adults who smoke and are not meeting recommended physical activity levels. Participants are randomly assigned to Actívatexto, which targets both smoking cessation and physical activity with text messages, wearable monitoring, nicotine replacement therapy, and an online dashboard, or to Decídetexto, which targets only smoking cessation. All participants receive nicotine replacement therapy and a Fitbit device, with assessments at baseline, 3 months, and 6 months. Recruitment is community-based and the research team manages incoming data from texts and wearables to support participants.
Who should consider this trial
Good fit: Ideal candidates are Latino adults (18+) who smoke at least three days per week, want to quit within 30 days, are insufficiently active, have a text-capable cellphone, and can wear a device daily and attend study visits.
Not a fit: People who use non-cigarette tobacco products, are pregnant or breastfeeding, plan to move away in six months, have another household member enrolled, or cannot safely increase activity are excluded and unlikely to benefit from this program.
Why it matters
Potential benefit: If successful, the program could provide a scalable way to help Latino smokers quit and become more physically active, lowering their long-term health risks.
How similar studies have performed: Standalone text-messaging programs and nicotine replacement therapies have shown success for smoking cessation, but integrated text-plus-wearable interventions that also target physical activity—especially among Latinos—are less well tested.

Inclusion Criteria: * Self-identify as Hispanic and/or Latino * Know how to read and speak English and/or Spanish * Be ≥ 18 years of age * Not meet the recommended levels of physical activity (150 minutes of moderate to vigorous physical activity) * Smoke cigarettes at least 3 days per week * Be interested in quitting smoking in the next 30 days * Have a cellphone with text messaging capability * Know how to utilize text messages * Be willing to wear a wearable device daily to monitor physical activity * Be willing to complete all study visits Exclusion Criteria: * Use of tobacco products other than cigarettes in the past 30 days (including e-cigarettes) * Being pregnant or breastfeeding * Plan to move from their current residence in the next six months * Another household member enrolled in the study * Unable to become more physically active or engage in a fitness appraisal
