What this trial studies
This research evaluates the effects of a specific physical activity training program, known as the GH Method, on patients undergoing dialysis or suffering from chronic kidney disease stages 4 and 5. The study will enroll 150 clinically debilitated patients who will be randomly assigned to either an intervention group, which will participate in a 12-month personalized exercise rehabilitation program, or a control group with no exercise intervention. Health and functional measures will be assessed at baseline, 6 months, and 12 months to determine the impact of the exercise program. Additionally, a subgroup of patients who are pre- or post-kidney transplant will also be included to assess the intervention's effects in this population.
Who should consider this trial
Good fit: Ideal candidates include adults aged 18 and older who are on dialysis or have chronic kidney disease stages 4 or 5 and are considered clinically debilitated.
Not a fit: Patients with cardiac or pulmonary diseases that contraindicate physical training or those unable to provide consent or travel to the training center may not benefit from this study.

Inclusion Criteria: * Clinically debilitated patients, currently on dialysis, or have CKD 4 (GFR between 15 and 29), or have CKD 5 (GFR below 15), or post- kidney transplant (for PPKT Subgroup only) as stated by patient's health care professional. * Adequate cognitive ability to complete the questionnaires, give consent for the study and follow the physical and diet instructions Exclusion Criteria: * cardiac/pulmonary disease that contraindicate the physical training * Unable to give consent * Unable to travel to the training center
