This project will follow adults who received a kidney transplant at Royal Derby Hospital to see how cardiovascular risk factors and other health conditions change over the first year.

What this trial studies
This observational project enrolls adults within one month of a single-organ kidney transplant at Royal Derby Hospital and follows them for the first post-transplant year. Investigators will collect routine clinical data and perform measurements including blood pressure, BMI, blood tests, body composition (muscle and fat), and physical fitness assessments, and will record new diagnoses. There are no experimental treatments; participation involves scheduled measurements and review of medical records. The goal is to describe patterns and progression of multimorbidity and changes in heart disease and stroke risk factors after transplant.
Who should consider this trial
Good fit: Adults aged 18 or older who can give informed consent, have received a single-organ kidney transplant and are followed up by University Hospitals of Derby and Burton (Royal Derby Hospital) are eligible within the first month after transplant.
Not a fit: Patients whose graft fails before they return to follow up, or those not followed by UHDB, are unlikely to benefit from this centre-based observational work.
Why it matters
Potential benefit: If successful, the findings could help clinicians identify common emerging health problems and modifiable cardiovascular risk factors so care can be targeted earlier for transplant recipients.
How similar studies have performed: Previous record-based analyses have shown a high burden of comorbidity at the time of transplant, but few studies have tracked multimorbidity progression and cardiovascular risk factors longitudinally after transplant.

Inclusion Criteria: * Age ≥ 18 years old * Capable of giving informed consent * In receipt of a single organ renal transplant and being followed up by UHDB as the referring team. Exclusion Criteria: • Graft failure before return to follow up
