Predicting extubation failure in ICU patients using a physiotherapy assessment
Use of a Physiotherapy Assessment to Predict Extubation Failure in Mechanically Ventilated Patients: the EPIC Assessment
This study is trying to see if a physiotherapy assessment can help predict which ICU patients on a ventilator might struggle after being taken off the machine.

What this trial studies
This observational study aims to identify predictive factors for extubation failure in mechanically ventilated patients in the ICU. Adult patients who have been on invasive mechanical ventilation for over 48 hours will undergo a spontaneous ventilation test, followed by an EPIC Assessment conducted by physiotherapists. The study will collect data on patient characteristics and known risk factors for extubation failure to enhance prediction accuracy. The goal is to improve decision-making regarding extubation and reduce associated complications.
Who should consider this trial
Good fit: Ideal candidates are adults aged 18 and older who have been on invasive mechanical ventilation for more than 48 hours and have passed a spontaneous ventilation test.
Not a fit: Patients who are tracheostomized, have severe psychiatric or cognitive disorders, or are under therapeutic restriction will not benefit from this study.
Why it matters
Potential benefit: If successful, this approach could significantly reduce the rates of extubation failure and its associated complications in ICU patients.
How similar studies have performed: While several studies have explored predictive criteria for extubation failure, this specific combined assessment approach is novel and has not been widely tested.
