This project will measure one-year survival and long-term outcomes in adults in China who become chronically critically ill after prolonged ICU stays.

What this trial studies

This observational cohort will enroll adults at West China Hospital who remain in the ICU eight days or longer and meet criteria for chronic critical illness (for example tracheotomy, prolonged mechanical ventilation, multi-organ failure, sepsis, severe trauma, or severe brain injury). Clinical data will be recorded during the ICU stay and researchers will conduct semi-structured telephone follow-ups to determine one-year mortality and other long-term outcomes. The study will analyze the epidemiology and identify risk factors associated with poor outcomes. No experimental treatments are given; the goal is to describe patterns of illness and outcomes to inform clinical care and future research.

Who should consider this trial

Good fit: Adults over 18 treated at the enrolling ICU who stay eight days or more and have tracheotomy, continuous mechanical ventilation >96 hours, multi-organ failure, sepsis, severe trauma, or severe brain injury, and who agree to telephone follow-up.

Not a fit: Patients with shorter ICU stays, those who refuse or cannot be reached for follow-up, or those with conditions unlikely to be altered by care-planning changes may not directly benefit.

Why it matters

Potential benefit: If successful, the results could help clinicians identify patients at highest risk and guide decisions about post-ICU care and follow-up.

How similar studies have performed: Similar cohort studies in other countries have documented high one-year mortality and persistent disability among chronically critically ill patients, but large-scale data from China remain limited.

Inclusion Criteria:

\- Must meet: Age greater than 18 years old; Stay in ICU for 8 days or more.

Meet any of the following criteria:

Tracheotomy; Continuous mechanical ventilation for over 96 hours; Multiple organ failure; Sepsis or severe infection; Severe trauma; Severe brain injury;

Exclusion Criteria:

\- Refusing to accept follow-up; Patients who have been lost to follow-up;
