This project will test a GP-based vision-screening program for people aged 70 and older with at least one chronic condition to find sight problems early.

What this trial studies
Using the Medical Research Council framework for complex interventions, investigators will co-develop a primary care intervention with patients, relatives, and health professionals to help GPs identify and coordinate care for older patients with low vision. The team will use qualitative and creative methods, including graphical facilitation, to design the model and will pilot its feasibility in real-world general practice settings. The main outcome is detection of vision impairment and the secondary outcome is identification of eye conditions needing ophthalmologic follow-up before vision loss occurs. The project aims to explore the potentials and limits of general practice in detecting preventable vision loss.
Who should consider this trial
Good fit: People aged 70 or older who have one or more chronic diseases, are followed by a Danish GP for their chronic condition, and can understand Danish would be the intended participants.
Not a fit: Patients with diagnosed dementia, those already under care by a private ophthalmologist or with known eye disease, and people who do not understand Danish are unlikely to benefit from this intervention.
Why it matters
Potential benefit: If successful, the program could help GPs detect treatable eye disease earlier, reducing preventable vision loss and preserving patients' independence and quality of life.
How similar studies have performed: Primary-care vision screening and referral pathways have been tried in other settings with mixed results, and large-scale evidence for lasting population benefit remains limited.

Inclusion Criteria: * +70 years * One or more chronic diseases * Are followed by GP du to chronic disease Exclusion Criteria: * Dementia diagnosis * Known eye-diseases or are currently followed by private ophtalmologist * Not able to understand Danish
