Diagnosed with chronic respiratory failure receiving any of:

* Non-invasive ventilation
* Prolonged mechanical ventilation
* Mechanical insufflation-exsufflation therapy

Exclusion Criteria:

* Aged \<18
* Significant physical or psychiatric co-morbidity that would prevent compliance with trial protocol
