Physical Activity in Persons With Parkinson's Disease - a Longitudinal Cohort Study
This project will follow about 450 people with Parkinson's over four years using wearable activity monitors, clinical tests, and questionnaires to see which physical, cognitive, social, or environmental factors make people less active.

What this trial studies
This is a national, multicentre longitudinal cohort that will enroll approximately 450 people with idiopathic Parkinson's disease across five Swedish regions, continuing an internal pilot. Physical activity will be measured objectively with accelerometers alongside repeated clinical assessments and digital questionnaires over four years. The primary outcome is change in accelerometer-measured physical activity, with exposure variables including physical, cognitive, disease-specific, social, environmental, motivational, and personal factors. Data collection combines clinical tests, patient-reported experience measures, and continuous activity-monitor data to identify patterns and predictors of declining activity.
Who should consider this trial
Good fit: Adults diagnosed with idiopathic Parkinson's disease at Hoehn & Yahr stages 1–4 who can complete the required physical and clinical assessments are ideal candidates.
Not a fit: People who are wheelchair bound or bedridden (Hoehn & Yahr stage 5) or who cannot perform the critical physical assessments would not be eligible and are unlikely to benefit from participation.
Why it matters
Potential benefit: If successful, the findings could identify modifiable barriers and supports so clinicians can offer personalized strategies to help people with Parkinson's stay active.
How similar studies have performed: Previous smaller studies using accelerometers in Parkinson's have shown feasibility and links between symptoms and activity, but large multicentre, long-term cohorts like this are relatively novel.

Inclusion Criteria: * People diagnosed with idiopathic Parkinson's disease * Hoehn \& Yahr 1 to 4 Exclusion Criteria: * Hoehn \& Yahr 5 (i.e. wheelchair bound or bedridden unless aided) * Unable to perform critical physical activity and clinical assessments
