What this trial studies

This observational database enrolls patients referred to the Prepare for Your Surgery program and systematically collects demographic, clinical, and perioperative data. Standardized frailty, cognitive, functional, and comorbidity instruments (for example CFS, FRAIL, FRIED, MoCA/Mini-Cog, Barthel, Charlson) are recorded along with hospitalization details and planned interventions. A nurse navigator carries out screening and coordinates a seven-stage preparation program delivered in person or online, with at least four weeks available before the scheduled elective operation. The dataset is intended to support tailored perioperative care and inform future interventions for frail surgical patients in Chile.

Who should consider this trial

Good fit: Adults referred to the program who are classified as frail (CFS ≥ 4, FRAIL, FRIED phenotype, or surgeon judgment) and have at least four weeks before elective surgery at Hospital Clínico Red de Salud UC-Christus.

Not a fit: Patients requiring emergency surgery, those having elective surgery at another hospital, or those unable to comply with the program interventions are unlikely to benefit from this program.

Inclusion Criteria:

* Patients referred to the Prepare for Your Surgery program
* Patients characterized as frail according to the Clinical Frailty Scale (score ≥ 4), FRAIL scale or FRIED frailty phenotype or the clinical judgment of the referring surgeon, with at least 4 weeks to receive surgery
* Elective surgery to be performed at Hospital Clinico Red de Salud UC-Christus

Exclusion Criteria:

* Patients who will not undergo elective surgery at Hospital Clínico Red de Salud UC-Christus.
* Patients who require emergency surgery
* Patients who for various reasons cannot comply with the interventions proposed for this preparation.
