What this trial studies

This observational study aims to establish a cohort of 1000 patients with chronic kidney disease (CKD) to evaluate the effectiveness of integrated diagnosis and treatment approaches in a real-world setting. Participants will undergo regular monitoring and management of various health parameters, including blood pressure, blood glucose, and anemia, while receiving guidance on diet and exercise. The study will also analyze the clinical prognosis of patients based on different risk stratifications and identify factors associated with cardiovascular disease outcomes.

Inclusion Criteria:

Exclusion Criteria:

* Age \< 18 or ≥100 years;
* Unable to comply with the procedures stipulated in this study;
* Estimated follow-up time ≤6 months.
