What this trial studies

This clinical trial aims to enhance patient engagement in the Collaborative Care Model (CoCM) for anxiety and depression by integrating patients' personal goals and concerns into the treatment process. Participants will complete pre-visit questionnaires that will be used to create a visual dashboard for clinicians, allowing for a more tailored and holistic approach to care. The study will compare the effectiveness of this enhanced collaborative care with traditional methods, focusing on patient and clinician satisfaction. By addressing the limitations of current CoCM practices, the trial seeks to improve treatment outcomes and patient motivation.
