We will see if an AI program can score surgeons' stitching technique during left anterior descending artery bypass in adults having their first isolated CABG.

What this trial studies

This observational project collects operative recordings and related imaging from adults undergoing internal mammary artery-to-left anterior descending artery bypass as part of first-time isolated CABG. An AI model will analyze features of the vascular anastomosis and generate proficiency scores for surgeons. Those AI scores will be compared with independent human expert evaluations to measure consistency. No changes to surgical care are made and data are gathered at a single cardiac center in Beijing.

Who should consider this trial

Good fit: Adults (≥18 years) having a first-time isolated CABG with an internal mammary artery-to-LAD graft who can provide written informed consent are the intended participants.

Not a fit: Patients with acute coronary syndrome, contraindications to coronary CT/angiography, or significant renal or liver impairment are excluded and are unlikely to benefit directly from participation, and participants should not expect direct therapeutic benefit because this is observational.

Why it matters

Potential benefit: If successful, this could help standardize measurement of surgical skill and improve training and quality monitoring, potentially leading to safer and more consistent CABG outcomes.

How similar studies have performed: Similar AI methods have shown promise for automatically scoring surgical skills in minimally invasive and simulated tasks, but applying them specifically to coronary artery bypass anastomosis is relatively novel.

Eligibility criteria

Inclusion Criteria:

* Age ≥ 18 years
* Undergoing internal mammary artery-to-left anterior descending artery bypass grafting
* First-time recipient of isolated CABG surgery
* Signed written informed consent

Exclusion Criteria:

* Patients with acute coronary syndrome
* Patients with contraindications to coronary CT angiography or coronary angiography
* Patients with renal insufficiency or active liver disease, including those with persistently elevated serum transaminases of unknown cause or any serum transaminase levels exceeding three times the upper limit of normal.
