What this trial studies

This multicenter prospective observational project will follow adult patients admitted to participating ICUs in Türkiye and record routinely collected clinical and laboratory data. SOFA and SOFA-2 scores will be calculated from data obtained during the first 24 hours of ICU admission and again at 72 hours for longer stays. No extra tests or treatments will be performed beyond standard clinical care. Patients will be followed until ICU discharge or death to measure the relationship between SOFA-2 scores and ICU mortality. The study will also compare SOFA-2 performance against the original SOFA score and account for differences across centers.

Who should consider this trial

Good fit: Adults aged 18 or older admitted to a participating intensive care unit in Türkiye who remain in the ICU for more than 24 hours (excluding pregnant patients and organ donors) are the intended candidates.

Not a fit: Patients under 18, pregnant patients, organ donors, those discharged or transferred within 24 hours, or those with unknown ICU outcomes are unlikely to benefit from this study's findings.

Why it matters

Potential benefit: If successful, SOFA-2 could give clinicians a more accurate way to predict ICU mortality and help guide triage and monitoring decisions.

How similar studies have performed: The original SOFA score is widely validated for ICU mortality prediction, while SOFA-2 is a newer update with limited external validation to date.

Inclusion Criteria:

* Adults aged 18 years or older
* Admission to a participating intensive care unit in Türkiye
* Intensive care unit stay longer than 24 hours

Exclusion Criteria:

* Patients younger than 18 years
* Pregnant patients
* Organ donors
* Patients with unknown intensive care unit discharge outcomes
