This project will follow people aged 60 and older in nursing homes and in home care to see if a new elderly care complex model leads to better health and more active-health awareness than home-based care.

What this trial studies

This observational cohort will enroll adults aged 60+ through nursing homes around the Tianjin Base and through home care settings. Investigators will collect health measures, functional status, and self-reported active-health behaviors and compare changes over time between centralized elderly care complexes and home health care. People who can consent and participate in physical examinations will be included, while bedridden or non-communicative individuals are excluded. The study aims to create a platform for selecting and promoting elderly care models suited to the Tianjin population.

Inclusion Criteria:

* Voluntarily participated in this study and provided signed informed consent forms.
* Both male and female.
* Age 60 years old and above.

Exclusion Criteria:

* Unable to participate in physical examinations due to being bedridden or having limited mobility.
* Unable to communicate effectively.
* People cannot participate for other reasons.
