What this trial studies

In a randomized 1:1 feasibility trial, 48 people with Parkinson's disease and symptom duration of four or more years will be assigned to usual care or to a 12-week supervised exercise program plus usual care. The intervention includes treadmill gait and dual-task motor-cognitive training, progressive gait and balance exercises, and progressive lower-body resistance training delivered three times per week for one hour by a physiotherapist or exercise physiologist. Outcomes will be measured at baseline, 12 weeks, and at 6- and 12-month follow-up to capture short- and longer-term effects. The protocol was developed with the Copenhagen Trial Unit and detailed training protocols will be shared to support reproducibility.

Who should consider this trial

Good fit: Adults (≥18 years) with Parkinson's disease by MDS criteria, symptom duration of at least 4 years, who can walk independently and can provide informed consent are ideal candidates.

Not a fit: People with Parkinson's disease dementia, those unable to walk independently, or with unstable medical issues are unlikely to benefit or may be ineligible.

Why it matters

Potential benefit: If successful, the program could improve gait and balance, help reduce fall risk, and preserve mobility for people with moderate-to-advanced Parkinson's disease.

How similar studies have performed: Previous exercise and dual-task training studies in Parkinson's disease have shown promising improvements in gait and balance, so this builds on encouraging but not definitive evidence.

* Diagnosis of PD according to MDS-PD criteria
* Aged ≥ 18 years
* Informed consent
* PD symptoms ≥ 4 years
* Independent gait

Exclusion Criteria:

* Diagnosis of PD Dementia according to the MDS-PD Dementia criteria
