What this trial studies
The Yale Heart Failure Registry is an electronic health records-based registry designed to monitor heart failure cases in real-time across the Yale New Haven Health System. This registry collects comprehensive data on patients diagnosed with heart failure, including demographics, comorbidities, laboratory results, imaging, medications, and clinical outcomes such as hospital readmissions. By updating daily, the registry aims to identify gaps in guideline-recommended therapies and improve the quality of care for heart failure patients through precision medicine.
Who should consider this trial
Good fit: Ideal candidates for this study are patients diagnosed with heart failure who are receiving care within the Yale New Haven Health System.
Not a fit: Patients under the age of 18 will not benefit from this study as they are excluded from participation.
Why it matters
Potential benefit: If successful, this registry could enhance the management and treatment of heart failure patients by providing insights into care gaps and improving adherence to clinical guidelines.
How similar studies have performed: Other studies utilizing electronic health records for monitoring chronic diseases have shown success, indicating that this approach is promising and not entirely novel.
