What this trial studies
This is a prospective, multicenter observational cohort that enrolls patients who survive cardiogenic shock to characterize their subacute course after hospital discharge. Participants will be followed with scheduled visits and data collection on clinical outcomes, healthcare use, and recovery to identify common challenges after discharge. The study aims to map care gaps and inform the development of a coordinated healthcare network to support safe return to the community. The design is observational and does not test an experimental treatment.
Who should consider this trial
Good fit: Adults (age 18 or older) who survive an episode of cardiogenic shock in hospital and agree to participate in follow-up are ideal candidates.
Not a fit: Patients who died during the hospital stay, are under 18, or who refuse follow-up are not eligible and would not receive benefit from participation.
Why it matters
Potential benefit: If successful, the findings could lead to better coordinated post-discharge care and support services that reduce complications and readmissions for survivors of cardiogenic shock.
How similar studies have performed: Epidemiology has documented ongoing burden, but there are relatively few prospective cohorts focused specifically on the post-discharge subacute needs of cardiogenic shock survivors, so this approach is comparatively novel.

Inclusion Criteria: Hospital survivors of cardiogenic shock Exclusion Criteria: * Age \< 18 years。 * Patient or family refuse follow-up. * Patient died within hospital stay.
