This project will collect health data to see how type 2 diabetes with obesity changes over time in adults aged 18–50 who can read Chinese and use a smartphone.

What this trial studies

Participants receive a structured baseline assessment by nurses guided by the JADE web portal, including eye and foot exams and blood and urine tests. Key measures collected include BMI, blood pressure, HbA1c, lipid profile, renal and liver function, and urine albumin–creatinine ratio. Patients return at three years for repeat physical and psychological health assessments to document changes over time. The register is observational and does not assign any experimental treatments.

Who should consider this trial

Good fit: Adults aged 18–50 with type 2 diabetes and obesity (BMI ≥27.5 kg/m2 or waist ≥80 cm in women/≥90 cm in men) who can read Chinese and use smartphone/web-based tools.

Not a fit: People with type 1 diabetes, active or recent cancer (less than 5 years disease-free), life expectancy under 12 months, or those unable to read Chinese or use smartphone technology are unlikely to benefit from participation.

Why it matters

Potential benefit: If successful, the register could help detect complications earlier and inform more personalized care and public-health planning for young adults with diabesity in Hong Kong.

How similar studies have performed: JADE-based registries and other diabetes cohorts in Asia have previously been used to improve risk stratification and care coordination, though the magnitude of benefit varies by program and setting.

Eligibility criteria

Inclusion Criteria:

1. Type 2 diabetes;
2. Obesity reaching action level as defined as body mass index (BMI) 27.5 kg/m2 and/or waist circumference (WC) 80cm in women and 90cm in men;
3. Age between 18 and 50 years;
4. Able to conversate with smartphone technology and web-based program.
5. Able to read Chinese and communicate using Chinese.

Exclusion Criteria:

1. Type 1 diabetes;
2. Active malignant disease including those with history of malignant disease less than 5 years of disease-free duration;
3. Life expectancy less than 12 months;
4. Any medical illness or condition as judged by the investigators as ineligible to participate the study.
