Examining blood pressure patterns in people at risk for heart disease
Diurnal Blood Pressure and Arterial Stiffness Patterns in Those at Increased Risk of Cardiovascular Disease
This study looks at daily blood pressure patterns in people at risk for heart disease, especially those with kidney issues, to see how these patterns might help identify who could benefit from specific treatments.

What this trial studies
This observational study aims to characterize diurnal blood pressure patterns in patients at increased risk of cardiovascular disease (CVD), particularly focusing on those with chronic kidney disease and other related conditions. It will assess 24-hour blood pressure and arterial stiffness to understand the implications of nocturnal blood pressure dipping and its association with CVD risk. The study also seeks to identify distinct patient groups who may benefit from targeted therapies based on their blood pressure patterns.
Who should consider this trial
Good fit: Ideal candidates include patients attending NHS Lothian services who are classified as at increased risk of CVD, including those with chronic kidney disease and other related conditions.
Not a fit: Patients under 18 or over 90 years old, or those unable to provide informed consent, may not benefit from this study.
Why it matters
Potential benefit: If successful, this study could lead to improved management strategies for patients at risk of cardiovascular disease, potentially reducing their risk of adverse events.
How similar studies have performed: While the approach of assessing diurnal blood pressure patterns is established, the specific focus on this patient cohort and the implications for chronic kidney disease is relatively novel.
Eligibility criteria

Inclusion Criteria: * Patients will be eligible to take part in the study if they attend NHS Lothian inpatient or outpatient services and can be classified as being at increased risk of CVD. This will include, but is not limited to, the following subgroups: 1. CKD as defined by the Kidney Disease Outcome Quality Initiative (K/DOQI) classification 2. AKI as defined by the Kidney Disease Improving Global Outcomes (KDIGO) classification 3. Small vessel vasculitis 4. Kidney transplant recipient 5. Kidney donor We will also recruit a healthy control group from the community. Exclusion Criteria: 1. Age \<18 years and \>90 years 2. Lack of ability to provide informed consent
