What this trial studies
This research evaluates the effectiveness of an electronic visit (e-visit) designed for smoking cessation in rural primary care settings in South Carolina. Participants will be randomly assigned to either receive the e-visit, which includes a questionnaire about their smoking habits and preferences for cessation medications, or to receive standard care. The study will involve collecting questionnaires and breath samples at four different time points over a 24-week period. The trial aims to assess both the effectiveness of the e-visit and its implementation in these rural clinics.
Who should consider this trial
Good fit: Ideal candidates are adult smokers aged 18 and older who smoke at least one cigarette per day and are willing to engage with digital health tools.
Not a fit: Patients currently undergoing cessation treatment or using FDA-approved cessation medications within the last week will not benefit from this study.

Inclusion Criteria: * Current cigarette smoking, defined as smoking 1+ cigarettes per day, for 20+ days out of the last 30, for the last 6+ months * Age 18+ * Enrolled in Epic's MyChart program or willing to enroll * Possess a valid e-mail address that is checked daily to access study assessments and MyChart messages * Owner of an iOS or Android compatible smartphone to provide remote CO readings * Have a valid address at which mail can be received (for mailing iCO™) * English fluency Exclusion Criteria: \- Current engagement in cessation treatment, defined as use of an FDA-approved cessation medication within the last 7 days
