ECHO Diabetes program for Federally Qualified Health Centers
Extension for Community Health Outcomes (ECHO) Diabetes Federally Qualified Health Centers (FQHCs)
This program will test whether a 6-month tele-education series for primary care teams at Federally Qualified Health Centers helps lower very high HbA1c rates and increases use of diabetes technology in adults with diabetes.

What this trial studies
The program uses a hub-and-spoke tele-education model delivering one-hour ECHO sessions twice monthly over six months to PCP teams at participating FQHCs, plus onboarding, real-time case support, and an online resource library. The multidisciplinary hub includes endocrinologists, CDCES, a psychologist, dietitian, exercise physiologist, PCPs, and health services researchers and offers CME credits. Eighteen spokes drawn from two Health Center Controlled Networks will be randomized into three staggered cohorts in a stepped-wedge design, with covariate-constrained randomization guiding assignment. Program effects will be measured using provider pre/post surveys and center-level aggregate clinical data, focusing on glycemic control and technology adoption.
Who should consider this trial
Good fit: Ideal participants are Federally Qualified Health Centers that are members of Health Choice Network or AllianceChicago and their adult patients with diabetes whose de-identified aggregate data can be shared for monitoring.
Not a fit: Patients served at clinics that are not FQHCs or not part of HCN or AllianceChicago, and those under 18 who are excluded from the adult aggregate datasets, are unlikely to benefit from this program.
Why it matters
Potential benefit: If successful, FQHC primary care teams could better manage diabetes so fewer patients have very high HbA1c and more patients gain access to CGM and insulin pumps.
How similar studies have performed: Project ECHO programs, including earlier ECHO Diabetes efforts, have generally improved provider knowledge and care processes, though patient-level outcome effects have been mixed across studies.

Inclusion Criteria: Federally Qualified Health centers (spokes) are eligible to participate that are part of Health Choice Network (HCN) or Alliance Chicago (AC). All people living with diabetes 18 and older are included in aggregate data sharing to evaluate outcomes. \- Exclusion Criteria: * Non FQHC, not part of HCN or AC
