What this trial studies
This mixed-methods pilot delivers the Chronic Disease Self-Management Program (CDSMP) in Hebrew to adults with chronic conditions recruited from a major tertiary center in Tel Aviv. Participants join weekly 2.5-hour online group sessions for six weeks led by certified facilitators, focusing on symptom management, action planning, medication adherence, physical activity, and communication with clinicians. The study collects validated self-report measures before the program, immediately after, and six months later, plus qualitative feedback to gauge feasibility and acceptability. Results will inform whether the CDSMP can be implemented more broadly for kidney transplant recipients and cancer survivors in Israel.
Who should consider this trial
Good fit: Ideal candidates are community-dwelling adults (age 21+) in Israel with at least one documented chronic condition, fluent in Hebrew, including kidney transplant recipients and cancer survivors.
Not a fit: People with documented cognitive impairment, major psychiatric disorders, recent hospitalization (past 3 months), or those not fluent in Hebrew or without reliable internet access are unlikely to benefit from this program.

Inclusion Criteria: * Age 21 years or older * Diagnosed with at least one chronic disease (as documented in the medical record) * Community-dwelling * Fluent in Hebrew Exclusion Criteria: * Documented cognitive impairment * Diagnosis of major depression or other psychiatric disorder (based on self-report or medical records) * Hospitalization in the past 3 months
