What this trial studies

This study aims to develop and evaluate the ASSIST 2.0 reablement program, which provides tailored support to older adults receiving home care. The program focuses on enhancing their ability to perform daily activities, boosting self-efficacy, and improving overall quality of life compared to standard home care services. Participants will receive assistance from home care staff, occupational therapists, and physical therapists, along with online education to empower them in their daily routines. The effectiveness of this program will be assessed through various measures of health and well-being.

Good fit: Ideal candidates for this study are older adults aged 65 and over who are receiving home care and can articulate their daily challenges.

Not a fit: Patients who are in palliative care may not benefit from this study.

Inclusion criteria:

* Homecare has been granted
* The person can describe an ordinary day and what they have problems with doing
* 65 years of age or over

Exclusion criteria:

• The person has palliative care
