What this trial studies
Researchers will screen older adults with COPD at Hospital 198 in Hanoi to determine how common frailty is in this population and to enroll eligible participants into an interventional comparison of the Vivifrail exercise program versus usual care. Participants who meet inclusion criteria (age ≥60, stable COPD, able to exercise) will follow a tailored Vivifrail exercise regimen while the control group receives usual care, with outcomes measured over the study period. Primary outcomes include physical performance, activities of daily living, and quality of life, with additional monitoring for safety and adverse events. The trial is conducted at the National Geriatric Hospital site within Hospital 198, Ministry of Public Security in Hanoi, Vietnam.
Who should consider this trial
Good fit: Ideal candidates are adults aged 60 or older with COPD by GOLD criteria who are clinically stable, physically able to take part in exercise, and can give informed consent.
Not a fit: Patients with recent COPD exacerbation, severe cardiovascular disease or musculoskeletal limitations, severe cognitive impairment, or those unable to participate in exercise are unlikely to benefit from this program.

Potential benefit: If successful, the program could improve strength, balance, daily functioning, and overall quality of life for older adults with COPD and frailty.
How similar studies have performed: Previous studies in general older populations have shown Vivifrail improves muscle strength and function, but evidence specifically in older adults with COPD and in Vietnam is limited.

Inclusion Criteria: * Age ≥ 60 years * Diagnosis of COPD according to GOLD criteria * Clinically stable COPD (no exacerbation within the past 4 weeks) * Able to participate in exercise training * Provide informed consent Exclusion Criteria: * Acute COPD exacerbation within the past 4 weeks * Severe cardiovascular disease limiting exercise * Severe cognitive impairment * Musculoskeletal disorders preventing exercise * Participation in another clinical trial
