Bridging Gaps, Enhancing Wellbeing: Examining the Effectiveness of the Technology-Enabled Collaborative Care for Diabetes and Mental Health (TECC-DM) Model
This program will test whether virtual health coaching with support from an interdisciplinary care team helps adults with type 2 diabetes and anxiety, depression, or diabetes distress manage their health better than usual care.

What this trial studies
The trial randomizes about 160 adults with type 2 diabetes and co-occurring mental health symptoms to receive either the TECC-DM intervention or usual care. TECC-DM combines weekly virtual health coaching calls, educational emails, online self-management tools, and support from an interdisciplinary care team, delivered in addition to existing diabetes care. The study measures patient-relevant outcomes such as quality of life and diabetes distress, as well as provider satisfaction, and includes parallel implementation research using input from a partner advisory group. Participants are enrolled and followed by a team led at McMaster University with collaborators including CAMH and Diabetes Canada.
Who should consider this trial
Good fit: Adults age 18 or older who live in Ontario, have diagnosed type 2 diabetes, and meet thresholds for anxiety/depression (HADS ≥8) or diabetes distress (DDS ≥2), and can communicate in English are ideal candidates.
Not a fit: People with severe mental illness (for example schizophrenia or psychotic disorders), those living outside Ontario, non-English speakers, or those without access to virtual services may not be eligible or receive benefit.
Why it matters
Potential benefit: If successful, the program could help people with type 2 diabetes and mental health symptoms improve self-management and quality of life through coordinated virtual support.
How similar studies have performed: Collaborative care and virtual coaching models have shown promise in prior studies and the TECC-DM team reports feasibility data, but the full effectiveness of this integrated virtual model is still being tested.

Inclusion Criteria: * Adults ≥18 years old * Diagnosed with type 2 diabetes (T2D) * Experiencing mental health challenges, defined as: * Hospital Anxiety and Depression Scale (HADS) score ≥8 on either subscale OR * Diabetes Distress Scale (DDS) score ≥2 * Community-living * Able to communicate verbally in English * Residing in Ontario during study participation Exclusion Criteria: * Diagnosis of severe mental illness (e.g., schizophrenia, psychotic disorder)
