This project will try a village-based program combining medicines, lifestyle support, education, and 7-day ECG monitoring for people aged 60+ in Qujiang with chronic heart conditions such as atrial fibrillation, high blood pressure, coronary artery disease, or heart failure.

What this trial studies

The BOUQUET cohort is a prospective, village-based observational study enrolling about 30,000 residents aged 60 and older in Qujiang District with chronic cardiovascular diseases. Participants are offered a comprehensive management program including standardized pharmacologic treatment, lifestyle modification counseling, health education, structured long-term follow-up, and enhanced monitoring with 7-day ECG recordings. Data on clinical events, medication adherence, functional status, and quality of life will be collected and linked to routine care records. Outcomes will be followed for up to 10 years to observe rates of major cardiovascular events, hospitalizations, and mortality in a real-world community setting.

Who should consider this trial

Good fit: Ideal candidates are Qujiang District residents aged 60 or older with a diagnosis of chronic cardiovascular disease (hypertension, coronary artery disease, atrial fibrillation, or heart failure) who can give informed consent and participate in long-term follow-up.

Not a fit: Patients with severe cognitive impairment, a non-cardiovascular life expectancy under one year, or those unwilling or unable to participate in regular follow-up are unlikely to gain benefit from this program.

Why it matters

Potential benefit: If successful, the program could lower heart-related hospitalizations and deaths and improve quality of life for older adults in rural communities.

How similar studies have performed: Integrated chronic disease management programs have previously improved risk factor control and reduced hospitalizations in older adults, though very large community-based cohorts with extended ECG monitoring are relatively uncommon.

Age ≥60 years

Resident of Qujiang District

Diagnosed with chronic CVD (hypertension, coronary artery disease, heart failure, atrial fibrillation, or others)

Able and willing to provide informed consent

Exclusion Criteria

Severe cognitive impairment or inability to provide consent

Life expectancy \<1 year due to non-cardiovascular disease
