What this trial studies

This is a randomized controlled trial enrolling 66 adults undergoing open nephrectomy under general anesthesia to compare conventional analgesia with an added ultrasound-guided erector spinae plane (ESP) block. Participants are randomized to receive standard postoperative pain management including opioids (control) or an ESP block plus general anesthesia (intervention). Primary outcomes are pain scores measured by visual analog scale (VAS) and total opioid consumption in the first 24 hours after surgery, with monitoring of vital signs and adverse events. The trial is conducted at Bangladesh Medical University and follows standard perioperative monitoring and follow-up.

Who should consider this trial

Good fit: Adults aged 18–64 with ASA physical status I–II scheduled for open nephrectomy who are not pregnant and have no contraindications to regional anesthesia are the intended participants.

Not a fit: Patients with allergy to local anesthetics, local infection at the block site, spinal anatomical deformities, coagulopathy, pregnancy or breastfeeding, or a history of severe chronic pain syndromes may not benefit or be eligible.

Why it matters

Potential benefit: If successful, the ESP block could provide better postoperative pain control with lower opioid requirements and fewer opioid-related side effects for nephrectomy patients.

How similar studies have performed: Prior reports and smaller trials of ESP blocks in thoracic and abdominal surgeries have shown reduced pain and opioid use, but high-quality randomized data specifically for open nephrectomy remain limited.

Inclusion Criteria:

* Adult patients aged 18 to 64 years
* Scheduled for open nephrectomy (radical or partial) under general anesthesia
* ASA physical status I-II (American Society of Anesthesiologists classification)

Exclusion Criteria:

* Allergy to local anesthetics.
* Local infection or inflammation at the site of the Block placement.
* Presence of anatomical deformities in the spine
* Coagulopathy or any bleeding disorder.
* Pregnancy or breastfeeding women.
* History of severe chronic pain syndrome
