This project will test whether the Turkish version of the Behavioral Indicators of Pain Scale (ESCID) can reliably detect pain in adults on mechanical ventilation who cannot speak.

What this trial studies

Researchers will enroll adult ICU patients who are intubated, mechanically ventilated, nonverbal, and hospitalized at least 24 hours to apply the ESCID during routine care. Trained clinicians will record behavioral indicators and compare measurements to establish reliability (consistency between observers and over time) and validity. Patients with major neurological, hemodynamic, or postoperative complications, those who are unresponsive (RASS -5), or with suspected brain death will be excluded. The work will be conducted at Goztepe Prof Dr Suleyman Yalcin City Hospital with collaboration from national academic partners.

Who should consider this trial

Good fit: Adults (18+) who are intubated, receiving mechanical ventilation, unable to speak (GCS <14), and have been in the ICU at least 24 hours are ideal candidates.

Not a fit: Patients who can self-report pain, those who are unresponsive (RASS -5) or brain-dead, and patients with certain cognitive, psychiatric, or neuromuscular conditions are unlikely to benefit from this scale.

Eligibility criteria

Inclusion Criteria:

* Individuals aged 18 years and older
* Patients who are intubated, receiving mechanical ventilation, and unable to establish verbal communication (Glasgow Coma Scale score \<14)
* Patients who have been hospitalized in the intensive care unit for at least 24 hours

Exclusion Criteria:

* Presence of postoperative complications
* Hemodynamic instability
* History of diagnosed cognitive or psychiatric disorders
* History of epilepsy or use of neuromuscular blocking agents
* History of spinal cord injury affecting motor function of all four extremities
* Individuals with a Richmond Agitation-Sedation Scale (RASS) score of -5 (unresponsive)
* Cases with suspected brain death
