What this trial studies
Adults with stage I–III gastrointestinal cancer who are planned for abdominal surgery will be offered a trimodal prehabilitation program that combines physical training, nutritional support, and motivational coaching. The physical component emphasizes graded aerobic and resistance exercises with flexibility and balance work, personalized after a baseline functional assessment and a motivational interview. Exercise targets follow WHO-style recommendations (progressive moderate-to-vigorous activity, 150–300 minutes/week when tolerated) and are adapted to the patient’s capacity; supervised and home-based elements are used. The intervention is delivered at Hospital Universitario Puerta de Hierro Majadahonda and excludes patients with metastatic disease or poor performance status.
Who should consider this trial
Good fit: Adults (18+) with non-metastatic stage I–III gastrointestinal cancer, ECOG 0–2, able to complete a mile-time test and attend preoperative sessions at the Madrid site.
Not a fit: Patients with metastatic disease, ECOG >2, inability to perform exercise testing, or those requiring urgent/emergency surgery are unlikely to benefit from this prehabilitation program.
Why it matters
Potential benefit: If successful, the program could improve patients' fitness before surgery, reduce postoperative complications, and shorten hospital recovery.
How similar studies have performed: Prior multimodal prehabilitation programs, particularly in colorectal and other abdominal surgery, have shown improvements in preoperative fitness and reduced complications, though findings vary between trials.

Inclusion Criteria: * More than 18 years old * ECOG 0-2 * Being able to complete the mile-time test * Stages I-III * Being able to understand the informed consent * Gastrointestinal cancer diagnosed Exclusion Criteria: * Metastatic cancer
