What this trial studies

This prospective, observational, multicenter registry will enroll adults diagnosed with ALS at major academic hospitals and neuromuscular centers in Thailand and follow them longitudinally using standardized assessments. Data collected will include ALSFRS-R scores, staging systems (King, MiTos, 9-point), motor and respiratory function tests, quality-of-life measures, cognitive and behavioral evaluations, genetic profiles, and treatments received. All data will be entered into a secure REDCap database with protections under Thai PDPA and international standards. No experimental interventions are provided; the registry is intended to define epidemiology and natural history and to serve as a platform for future clinical and translational research.

Who should consider this trial

Good fit: Adults (age ≥18) with a diagnosis of ALS by El Escorial or Gold Coast criteria who are willing and able to provide informed consent and receive care at participating Thai centers are ideal candidates.

Not a fit: Patients who decline consent, who have alternative diagnoses that mimic ALS, or who expect direct access to experimental treatments should not expect clinical benefit from registry participation.

Why it matters

Potential benefit: If successful, the registry could improve understanding of ALS in Thailand, help optimize patient care, and make it easier to run future clinical trials locally.

How similar studies have performed: National and international ALS registries have previously yielded important epidemiological, genetic, and natural-history insights, so the registry approach is established though large-scale Thai data are currently lacking.

Inclusion Criteria:

* Diagnosis of ALS according to El Escorial or Gold Coast criteria
* Age ≥ 18 years
* Ability and willingness to provide informed consent

Exclusion Criteria:

* Patients unwilling to provide informed consent
* Patients with alternative diagnoses mimicking ALS
