What this trial studies
The project uses an MRC-inspired complex intervention framework and a patient-and-family participatory design to build a tailored telerehabilitation model. First, the team will co-create the intervention with patients and families; second, they will run a quasi-experimental comparison between the telerehabilitation group and a usual-care control group; third, they will conduct qualitative participant observation and interviews to explore patient experience. The intervention is delivered via video consultations and home monitoring and targets outcomes such as health literacy, health-related quality of life, family support, and communication with outpatient nurses. Quantitative measures and descriptive qualitative methods will be combined to understand both effect and experience.
Who should consider this trial
Good fit: Adults attending outpatient cardiac rehabilitation at Amager and Hvidovre Hospitals with ischemic heart disease, heart failure, persistent atrial fibrillation, or post-cardiac valve surgery who can use a smartphone, tablet, or computer and communicate in the study language.
Not a fit: Patients with substantial language barriers, significant cognitive impairment, or inability to use digital devices are unlikely to benefit from this telemedicine approach.
Why it matters
Potential benefit: If successful, the program could improve patients' understanding of their condition, strengthen family support, and make rehabilitation more convenient through remote care.
How similar studies have performed: Previous cardiac telerehabilitation studies have shown comparable clinical and quality-of-life outcomes and improved access, but a tailored, patient-and-family co-created model like this remains relatively novel.

Inclusion Criteria: * Patients and their family members affiliated with the Department of Cardiology at Amager and Hvidovre Hospital and attending cardiac rehabilitation. * Patients diagnosed with ischemic heart disease, heart failure, persistent atrial fibrillation, and cardiac valve surgery. Exclusion Criteria: * Patients with substantial language barriers and limited cognitive function. * Patients who can't use a smart phone, tablet, or computer.
