What this trial studies

This interventional stepped-wedge hybrid type II trial implements a train-the-trainer Quality Improvement program called TeamBirth on labor and delivery units to introduce patient-participatory shared decision-making huddles. Sites transition from usual care to the TeamBirth program on a randomized schedule, allowing comparison of outcomes before and after implementation. Key outcomes include rates of nulliparous term singleton vertex (NTSV) cesarean birth, measures of shared decision-making, and patient-reported respectful care and experience, with attention to equity for Black birthing people. Collaborators include University of Illinois at Chicago, University of Chicago, and Ariadne Labs, with Endeavor Health as lead sponsor.

Who should consider this trial

Good fit: Ideal candidates are birthing people aged 18 or older who labor and deliver a live singleton infant and speak English or Spanish.

Not a fit: People under 18, non-English/non-Spanish speakers, those who had a cesarean without labor, or those who gave birth to a nonliving infant would not be eligible and are unlikely to receive benefit from this program.

Inclusion Criteria:

* Age 18 or older
* English or Spanish speaking
* Gave birth to a live-born infant after laboring

Exclusion Criteria:

* Speaks a language other than English or Spanish
* Under the age of 18
* Gave birth to a nonliving infant
* Cesarean delivery without labor
