TeamBirth shared decision-making huddles to improve safety, experience, and equity during labor
Improving Safety, Patient Experience, and Equity Through Shared Decision-making Huddles in Labor (I'M SPEAKING)
This project tests whether training labor teams in the TeamBirth shared decision-making huddle reduces first-time cesarean births and improves respectful care and patient experience for people laboring for the first time, including Black birthing people.

What this trial studies
This interventional stepped-wedge hybrid type II trial implements a train-the-trainer Quality Improvement program called TeamBirth on labor and delivery units to introduce patient-participatory shared decision-making huddles. Sites transition from usual care to the TeamBirth program on a randomized schedule, allowing comparison of outcomes before and after implementation. Key outcomes include rates of nulliparous term singleton vertex (NTSV) cesarean birth, measures of shared decision-making, and patient-reported respectful care and experience, with attention to equity for Black birthing people. Collaborators include University of Illinois at Chicago, University of Chicago, and Ariadne Labs, with Endeavor Health as lead sponsor.
Who should consider this trial
Good fit: Ideal candidates are birthing people aged 18 or older who labor and deliver a live singleton infant and speak English or Spanish.
Not a fit: People under 18, non-English/non-Spanish speakers, those who had a cesarean without labor, or those who gave birth to a nonliving infant would not be eligible and are unlikely to receive benefit from this program.
Why it matters
Potential benefit: If successful, the intervention could lower unnecessary first-time cesarean births and improve respectful, equitable decision-making and overall patient experience during labor.
How similar studies have performed: Previous quality-improvement and shared decision-making initiatives, including earlier pilot work with TeamBirth and related communication interventions, have shown promise in improving communication and reducing unnecessary interventions, though large randomized evidence remains limited.
