What this trial studies
This intervention focuses on providing peer support to individuals diagnosed with psychotic disorders, including schizophrenia and bipolar disorder, who have recently been discharged from psychiatric facilities. Community Mental Health Workers and Peer Support Workers will deliver a structured program aimed at helping participants build connections to support systems and navigate their recovery journey. Inclusion Criteria: * 18-65 years of age. * Any psychotic disorder diagnosis based on the International Classification of Diseases - 10th revision (ICD-10) criteria, including both non-affective (e.g., schizophrenia) and affective psychosis (e.g., bipolar disorder) * Having been discharged from inpatient psychiatric treatment facilities in the month prior to recruitment Exclusion Criteria: * Active suicidal ideation. * Cognitive, neurological, or other sensorial conditions likely to preclude or affect an objective assessment via interview procedures
