This project will test whether cancer survivors in Metro Detroit can complete a social needs screening and be connected to community resources for food and digital access.

What this trial studies

Participants with a past or current cancer diagnosis who live in Metro Detroit will complete a social needs screening either in person or by telephone. Responses will be translated into diagnosis codes that identify non-medical factors affecting health, and participants will be referred to community-based resources based on identified needs. The program includes brief interventions focused on food access and digital inclusion and follow-up telephone calls at 2, 4, 6, and 8 weeks to track referrals and outcomes. The team aims for at least an 80% completion rate and will use findings to refine referral procedures.

Who should consider this trial

Good fit: Adults (18+) with a past or current cancer diagnosis who live in Wayne, Oakland, or Macomb counties, are English proficient, and receive care at Karmanos outpatient clinics or are ROCS participants with prior food insecurity are ideal candidates.

Not a fit: Individuals who live outside the specified Metro Detroit counties, are not English proficient, or are not engaged with Karmanos care or the ROCS cohort are unlikely to qualify or benefit from this program.

Why it matters

Potential benefit: If successful, more survivors could be identified and linked quickly to food, internet access, and other community supports that help them manage health after cancer.

How similar studies have performed: Other health-system programs that screen for social determinants and make referrals have successfully identified needs and increased resource connections, though long-term impact on clinical outcomes remains limited.

Eligibility criteria

Inclusion Criteria:

* 18 years or older
* past or current cancer diagnosis
* reside in metropolitan Detroit (Wayne, Oakland, Macomb counties)
* English proficient
* Actively receiving care through one of Karmano's Cancer Institute's outpatient clinics OR post treatment survivors in the Detroit Research on Cancer Survivors (ROCS) longitudinal cohort study who have reported past food insecurity and agreed to be contacted for further research
