What this trial studies
Participants aged 16–29 with CKD stages 1–5 who own a smartphone will be offered a digital application designed to prompt and support guideline-recommended care and transitional needs. The intervention will be delivered through the St. James's Hospital Young Adult Clinic with coordination from Trinity College Dublin, and use will be tracked alongside clinical data. Outcomes include alignment with KDIGO 2024 recommendations, measures of adherence, and clinical markers such as blood pressure control and proteinuria management. A planned subgroup will specifically examine young people transferring from paediatric to adult services during the co-location of Children's Health Ireland at the St. James's campus.
Who should consider this trial
Good fit: Ideal candidates are 16–29-year-olds with CKD stages 1–5 who own a smartphone, can provide informed consent, and are engaged with or transitioning through the SJH Young Adult Clinic.
Not a fit: People aged 30 or older, those without smartphone access, or those with neurocognitive impairment preventing informed consent or app use are unlikely to benefit from this intervention.
Why it matters
Potential benefit: If successful, the app could increase adherence to guideline-recommended care, improve blood pressure and proteinuria management, and reduce long-term kidney and cardiovascular risks for young adults with CKD.
How similar studies have performed: Digital interventions in adult kidney disease have shown mixed improvements in guideline adherence and blood pressure control, while transition-focused digital tools for young adults remain relatively understudied.

Inclusion Criteria: * Adults aged ≥16 years. * Diagnosed with CKD stages 1-5. * Owns a smartphone and is capable of using mobile applications. * Provides informed consent. Exclusion Criteria: * Inability to provide informed consent due to a neurocognitive impairment. * Age 30 years or older The study will be conducted in the already established SJH Young Adult Clinic, with anticipated expansion coinciding with the co-location Children's Health Ireland (CHI) on site.
