What this trial studies
QUAD-HF is a prospective observational registry enrolling adults in the United Kingdom who receive a new diagnosis of heart failure with left ventricular ejection fraction (LVEF) <50% within 12 weeks. The registry will collect real-world data on diagnoses, treatments, guideline implementation, and service access across participating centers. Participants will be followed longitudinally for clinical outcomes such as hospitalizations and mortality. The goal is to characterize care variation and links between treatment use and outcomes in an unselected patient population.
Who should consider this trial
Good fit: Adults (over 18) in the UK newly diagnosed with heart failure and an LVEF below 50% within the prior 12 weeks who can give informed consent are the ideal candidates.
Not a fit: People with preserved LVEF (≥50%), those receiving palliative care, or those unable or unwilling to consent (or outside the UK) are unlikely to benefit from participation.
Why it matters
Potential benefit: If successful, this registry could identify gaps in real-world care and guide changes that reduce hospitalizations and improve survival for people with reduced LVEF heart failure.
How similar studies have performed: Other national and regional heart failure registries have successfully identified care gaps and informed policy and practice, and this registry applies that approach specifically to newly diagnosed patients with LVEF <50% in the UK.

Inclusion Criteria: 1. Age \>18 years 2. Newly diagnosed Heart Failure with a Left Ventricular Ejection Fraction \<50% (≤12 Weeks prior to enrolment). 3. Able and willing to consent. Exclusion Criteria: 1. LVEF 50% or more at time of diagnosis 2. Palliative care 3. Unable or unwilling to give consent
