What this trial studies
BP-REACH is a clinical trial designed to evaluate a team-based program involving pharmacists and health coaches aimed at reducing blood pressure in individuals with a history of stroke or myocardial infarction. The study compares this intervention to usual care within the Los Angeles Department of Health Services healthcare system. Participants will be randomized to either the intervention or usual care, with outcomes measured at baseline, 3 months, and 12 months, focusing on changes in systolic blood pressure. The intervention includes a home blood pressure monitor, monthly coaching calls, and medication management by a clinical pharmacist.
Who should consider this trial
Good fit: Ideal candidates include patients aged 35 and older with a history of ischemic or hemorrhagic stroke or myocardial infarction and a systolic blood pressure of 130 mm Hg or higher.
Not a fit: Patients under 35 years old, those with a systolic blood pressure below 130 mm Hg, or individuals who cannot provide informed consent due to cognitive impairments will not benefit from this study.

Inclusion Criteria: * Patients of LA General Medical Center, Rancho Los Amigos, Olive Medical Center or Harbor-UCLA Medical Center with ischemic stroke, hemorrhagic stroke, or myocardial infarction * English or Spanish speaking * Systolic blood pressure (SBP) ≥ 130 mm Hg Exclusion Criteria: * Age \< 35 years * SBP \< 130 mm Hg * Only speaks a language other than English or Spanish * Cannot provide informed consent due to dementia or aphasia
