What this trial studies
Participants will use the Care4Seniors 2.0 mobile application for guided physical and cognitive exercises and attend weekly in-person group exercise and cognitive sessions at community centres. The intervention combines intensive aerobic and resistance training with brain-health activities and lifestyle support to reduce frailty risk and social isolation. The program targets community-dwelling older adults with at least one chronic disease and is delivered over 12 weeks. Outcomes will focus on physical function, cognitive measures, and measures of social bonding and isolation.
Who should consider this trial
Good fit: Ideal candidates are community-dwelling adults aged 60–85 with at least one chronic disease who can communicate in English or Mandarin and can use a smartphone and attend weekly sessions.
Not a fit: People with severe vision or hearing impairment, diagnosed severe cognitive impairment (such as dementia), or severe psychiatric disorders are unlikely to benefit from this program.
Why it matters
Potential benefit: If successful, the program could slow or prevent frailty and cognitive decline while improving social connection and self-management in older adults.
How similar studies have performed: Similar multimodal programs combining exercise and cognitive training, sometimes supported by digital tools, have shown modest improvements in physical function and cognition in older adults, although results across studies are variable.

Inclusion Criteria: 1. Older Adults Aged 60 to 85 years old 2. Living in the community 3. Formally diagnosed with at least one chronic disease 4. Able to communicate in either English or Mandarin Exclusion Criteria: 1. Severe vision or hearing impairment 2. Diagnosed with severe cognitive impairment (e.g. dementia) 3. Diagnosed with severe psychiatric disorders (e.g. psychotic disorders, major depressive disorder)
