This program will test whether a four-week virtual Pain Resilience Promotion for Youth (PREP-Y) course can help teens ages 12–17 with chronic musculoskeletal pain become more physically active and cope better with their pain.

What this trial studies

PREP-Y is a four-week virtual intervention that teaches resilience skills such as motivation, confidence building, and pain coping to adolescents with chronic musculoskeletal pain. Participants complete surveys and physical activity tests before and after the program and wear activity monitors to track daily movement. The trial is interventional and focuses on feasibility, acceptability, and preliminary changes in activity and pain-related function. Findings will inform refinement of the program and planning for a larger follow-up trial.

Who should consider this trial

Good fit: Ideal candidates are English-speaking adolescents aged 12–17 with a primary chronic musculoskeletal pain disorder lasting more than three months, identified by a pediatric rheumatologist or pain physician.

Not a fit: Patients whose pain is secondary to another disease (such as sickle cell disease or inflammatory rheumatic disease), those with untreated major psychiatric disorders, or those with developmental delay are excluded and unlikely to benefit from this program.

Why it matters

Potential benefit: If successful, PREP-Y could help teens increase daily activity and reduce pain-related disability by teaching practical motivation and coping skills.

How similar studies have performed: Behavioral and physical-activity interventions, including cognitive-behavioral approaches, have shown benefits for pediatric chronic pain, but PREP-Y's brief virtual resilience-focused format is relatively new and less studied.

Eligibility criteria

Inclusion Criteria:

* Between the ages of 12 and 17
* History of chronic musculoskeletal pain for more than 3 months assessed by a pediatric rheumatologist or pain physician
* Diagnosis of a primary pain disorder (e.g., juvenile fibromyalgia and other widespread pain conditions, lower back pain, complex regional pain and amplified pain syndrome)
* Speaks English as primary language

Exclusion Criteria:

* Chronic pain secondary to disease (e.g., sickle cell disease or rheumatic disease such as juvenile arthritis, systemic lupus erythematosus)
* Presence of an untreated major psychiatric diagnosis (e.g., major depression, bi-polar disorder, psychoses)
* History or diagnosis of developmental delay.
