This program will test whether nurse-led foot care education based on the Health Belief Model helps adults with diabetes on hemodialysis improve foot care and lower their risk of foot ulcers.

What this trial studies

This randomized controlled pretest-posttest study will compare a Health Belief Model–based foot care education program delivered by hemodialysis nurses to usual care among diabetic patients receiving chronic hemodialysis. Fifty participants (25 per arm) recruited from three dialysis centers in Elazığ, Turkey will be randomized to the education or control group. Outcomes will include foot-care knowledge, self-care behaviors, and indicators related to ulcer risk measured before and after the intervention. The intervention is designed to provide practical skills and motivational resources to support daily foot-care practices and reduce complications.

Who should consider this trial

Good fit: Adults (age 18 or older) with diabetes who are receiving continuous hemodialysis, are literate, have had diabetes for at least six months, can communicate and cooperate, and agree to participate are ideal candidates.

Not a fit: Patients receiving acute hemodialysis, those under 18, people unwilling to participate or who have severe communication or psychiatric problems are excluded and unlikely to benefit from this intervention.

Why it matters

Potential benefit: If successful, the program could increase foot-care knowledge and self-care behaviors and potentially reduce the incidence of foot ulcers and lower-limb amputations in this high-risk group.

How similar studies have performed: Health Belief Model–based and nurse-led foot-care education has shown benefits in other diabetic populations, but similar interventions in chronic hemodialysis patients are limited and have not been widely tested.

Inclusion criteria:

* Voluntary participation in the study
* Literacy
* Absence of psychiatric distress
* Being over 18 years of age
* Open to communication and cooperation, no speech problems
* Receiving continuous hemodialysis treatment
* Having been diagnosed with diabetes for at least 6 months

Exclusion criteria:

* Unwillingness to participate in the study or desire to withdraw from the study
* Receiving acute hemodialysis treatment
* Being under 18 years of age
* Open to communication and cooperation, no speech problems
