Nurse-assisted eHealth intervention for patients with heart failure and colon-rectal cancer

What this trial studies
This randomized controlled trial aims to assist patients with heart failure and colon-rectal cancer through a 42-day nurse-assisted eHealth intervention called 'eHealth@ Hospital-2-Home'. Patients will be recruited before hospital discharge and will receive support in monitoring vital signs, reporting symptoms, and accessing health resources, while maintaining communication with a Nurse Navigator. The control group will receive standard care, and data will be collected at baseline, post-intervention, and six months later to assess the effectiveness of the intervention.
Who should consider this trial
Good fit: Ideal candidates include patients with symptomatic heart failure or those who have undergone surgery for colon or rectal cancer, who can communicate in Norwegian.
Not a fit: Patients with advanced heart failure requiring a transplant, those with metastatic cancer, or individuals facing acute medical crises may not benefit from this intervention.

Inclusion Criteria: * All participants: symptomatic HF or surgically treated for either colon or rectal cancer (Colon-Rectal Cancer Duke's class 1-3, curative), able to speak and write Norwegian Exclusion Criteria: * Heart failure population: patient is on a waiting list for a heart transplant, requires a Left Assist Ventricular Device (LVAD), and has a life expectancy \<6 months * Colon-rectal cancer population: metastatic cancer, Surgical Complication Score \> 3, and acute medical crisis.
