This study will test whether the modified thoracolumbar interfascial plane block (mTLIP) or the quadro-iliac plane block (QIPB) provides better pain relief for adults having lumbar microdiscectomy surgery.

What this trial studies

This randomized clinical trial will enroll 80 adults (18–65 years, ASA I–III) undergoing elective lumbar microdiscectomy and randomize 40 patients to bilateral ultrasound-guided mTLIP and 40 to bilateral QIPB performed before surgery. The primary outcome is patient-controlled opioid consumption during the first 48 postoperative hours; secondary outcomes include QoR-15 recovery score, static and dynamic NRS pain scores, total rescue analgesic use and time to first rescue, length of hospital stay, perioperative remifentanil dose, and block- or opioid-related side effects. All participants will receive standardized general anesthesia and perioperative care with prospective outcome collection through hospitalization. The trial directly compares an established fascial plane block (mTLIP) with a recently described alternative (QIPB) to determine relative safety and analgesic effectiveness.

Who should consider this trial

Good fit: Adults aged 18–65 with ASA physical status I–III scheduled for elective lumbar microdiscectomy who can provide informed consent.

Not a fit: Patients with prior lumbar spine surgery, diabetes, significant renal or hepatic impairment, bleeding risk, allergy to local anesthetics, pregnancy, or inability to reliably report pain are excluded and unlikely to benefit from this protocol.

Inclusion Criteria:

* Age between 18 and 65 years
* Classified as American Society of Anesthesiologists physical status I-III
* Scheduled for elective lumbar microdiscectomy surgery under general anesthesia
* Provision of written informed consent

Exclusion Criteria:

* Use of anticoagulant medications or presence of bleeding diathesis
* Known allergy or hypersensitivity to local anesthetics or opioid drugs
* Infection at the planned block site
* Alcohol or drug dependence
* Cognitive impairment preventing reliable pain assessment
* Pregnancy or lactation
* History of previous lumbar spine surgery
* Diabetes mellitus
* Renal or hepatic insufficiency
