What this trial studies
This single-site interventional protocol enrolls patients with glaucoma who previously underwent combined phacoemulsification with GATT or phacogoniopuncture and have persistent uncontrolled intraocular pressure. Participants will receive a modified trabeculectomy procedure that incorporates techniques such as deep non-penetrating sclerectomy, viscocanalostomy, and a small-incision trabeculectomy that avoids Tenon's capsule. The approach is intended to provide an alternative when ab-interno MIGS approaches have failed or are unsuitable, with postoperative follow-up to monitor intraocular pressure and complications. Outcomes will focus on pressure lowering, need for medications or further surgery, and procedure-related adverse events.
Who should consider this trial
Good fit: Adults with glaucoma who previously had combined phacoemulsification with GATT or phacogoniopuncture and continue to have inadequate intraocular pressure control are the intended candidates.
Not a fit: People who never had phaco with GATT or phacogoniopuncture, healthy individuals without glaucoma, or patients whose glaucoma is well controlled on current therapy are unlikely to benefit from this procedure.
Why it matters

Inclusion Criteria: * All patients had combined phaco surgery with GATT. * All patients had phaco with goinopuncture. Exclusion Criteria: * Healthy patient. * No history of phaco surgery with GATT or goinopuncture.
