What this trial studies
The project develops and implements an automated risk assessment and brief cognitive screening app (RACS) tailored for use in primary care settings. The tool is designed to minimize time and effort for clinicians or staff while remaining sensitive to cognitive decline across diverse educational and racial/ethnic backgrounds. The study will enroll Spanish‑fluent adults aged 60+ without a prior dementia diagnosis who can use an iPad and will be conducted at several UT‑affiliated primary care and geriatric clinics in Texas. Key outcomes include feasibility, screening rates, and identification of cognitive impairment during routine visits.
Who should consider this trial
Good fit: Ideal candidates are Spanish‑fluent adults aged 60 or older without a prior dementia diagnosis who can independently use an iPad and attend participating UT clinics in Texas.
Not a fit: People with a prior dementia diagnosis, significant motor or visual impairments that prevent independent iPad use, or who are not Spanish‑fluent are unlikely to benefit from this project.

Inclusion Criteria: * Inclusion Criteria: * Aged 60 years and older; * Ethnic/racial background consistent with NIH policy * Male or female * Fluent in Spanish. Exclusion Criteria: * Confounding conditions that could impact ability to participate in the study (e.g., cognitive impairment sufficient to impact ability to follow instructions on the iPad, motor impairment that would prohibit independent use of RACS, poor visual acuity) * Prior diagnosis of dementia * Non-Spanish speaking.
