Lifestyle Medicine: Establishing Clinical Approaches to Chronic Disease for Rural Patients
This program will try supervised exercise and lifestyle coaching for rural adults aged 18–64 who have two or more chronic conditions to see if access to a wellness facility improves their health.

What this trial studies
Adults who receive a physician referral from the Robert C. Byrd Clinic will enter a one-month referral window followed by three months of access to a supervised wellness facility as an adjunct to usual care. Participants will complete an intake visit to collect vitals, body measurements, medical history, physical activity readiness, and personal goals before beginning supervised physical activity and lifestyle coaching. Program entry is limited to adults 18–64 with two or more chronic diseases for which exercise therapy has evidence of benefit. Recruitment relies on primary care referrals and local outreach to engage rural patients.
Who should consider this trial
Good fit: Ideal candidates are adults 18–64 in the RCBC catchment area with a physician referral and at least two qualifying chronic diagnoses such as obesity, hyperlipidemia, PCOS, hypertension, coronary heart disease, or related conditions.
Not a fit: Patients without a physician referral, without qualifying chronic disease diagnoses, unwilling or unable to participate in supervised physical activity, or whose conditions lack evidence for exercise therapy are unlikely to benefit.
Why it matters
Potential benefit: If successful, the program could increase physical activity, improve body composition, and help control multiple chronic conditions among rural patients.
How similar studies have performed: Many clinical trials have shown that exercise and lifestyle programs improve cardiometabolic outcomes, but few have demonstrated implementation and uptake specifically in rural populations.

Inclusion Criteria: * Program admittance will be restricted to patients with 2+ diagnosed diseases that have sufficient evidence for the efficacy of exercise therapy (obesity, hyperlipidemia, metabolic syndrome, polycystic ovarian syndrome, type II diabetes, hypertension, coronary heart disease, heart failure, depression, anxiety) * physician referral required Exclusion Criteria: * no chronic disease diagnosis, lack of physician referral, unwillingness to participate.
