This project will test whether an electronic alert plus either the usual dental team or an added care navigator helps dental teams offer HIV testing and prevention to dental patients.

What this trial studies
The study uses an electronic Best Practice Alert (BPA) linked to an order set to prompt dental residents and staff to offer routine HIV screening and PrEP education during dental visits. Three urban general dentistry programs in New York City will be randomized in a crossover design to periods with the standard dental care team and periods with an enhanced team that includes a dedicated care navigator. Investigators will compare offer and uptake rates of HIV testing, linkage to results and prevention services, and implementation outcomes such as acceptability and feasibility among dental team members. Data will be collected from electronic health records and staff feedback to determine which model better supports consistent delivery of HIV prevention and care practices.
Who should consider this trial
Good fit: Ideal participants are dental team members (providers, hygienists, dental assistants, or care navigators) with direct patient contact at one of the three participating New York City dental programs who can provide written informed consent.
Not a fit: Patients who do not receive care at the participating clinics, who decline HIV testing, or who live outside these sites are unlikely to benefit directly from this intervention.
Why it matters
Potential benefit: If successful, this approach could increase routine HIV screening in dental settings and speed diagnosis and linkage to prevention and care, including PrEP.
How similar studies have performed: Electronic clinical decision support and opt-out HIV screening have increased testing in medical settings, but integration into dental clinics and the specific addition of care navigators is less well studied.

Inclusion Criteria: * Member of the Dental Team (Provider, Hygienist, Dental Assistant, Care Navigator) at one of the randomized sites with direct patient contact * Able to provide written informed consent. Exclusion Criteria: \- Inability to provide written informed consent.
