What this trial studies
Cleveland Clinic is scaling an intensive hypertension management program across 56 primary care practices in Northeast Ohio to reach roughly 3,800 patients over 48 months. The program expands team-based care—primary care providers working with pharmacists and advanced practice providers—to titrate blood pressure medicines using patient-recorded home blood pressure measurements. Implementation will include a 12-month pre-implementation phase with practice assessments, clinical decision support creation, staff training, and patient outreach planning. Quantitative outcomes will come from electronic medical records and surveys while qualitative methods (interviews, observations, reflections) will evaluate barriers and facilitators.
Who should consider this trial
Good fit: Adults 18–85 with a diagnosis of hypertension and uncontrolled blood pressure (greater than 150/95) who are cared for in Cleveland Clinic Northeast Ohio primary care practices and are not pregnant, not in stage 5 CKD/ESRD, and not in hospice.
Not a fit: Patients who are pregnant, have stage 5 chronic kidney disease or end-stage renal disease, are enrolled in hospice, or whose blood pressure is already controlled (≤150/95) are not expected to benefit from this program.
Why it matters
Potential benefit: If successful, the program could substantially increase the number of patients reaching blood pressure targets and lower their risk of heart attack and stroke.
How similar studies have performed: This program builds on prior randomized trial evidence (e.g., Margolis et al.) showing that pharmacist-led, home blood pressure–guided medication titration can improve blood pressure control.

Inclusion Criteria: * Hypertension diagnosis, aged 18-85 years old * Uncontrolled blood pressure, defined as a blood pressure reading of \> 150/95 Exclusion Criteria: * pregnant, stage 5 chronic kidney disease, End Stage Renal Disease, enrolled in hospice care
